1. Relationship to Our HIPAA Notice of Privacy Practices

This Privacy Policy applies to information collected through our website and online interactions. It does not govern the protected health information (PHI) we collect and maintain as part of the treatment relationship.

The way we handle your health information as a patient is described separately in our Notice of Privacy Practices (NPP), provided in accordance with the Health Insurance Portability and Accountability Act (HIPAA). If any conflict exists between this Privacy Policy and our Notice of Privacy Practices with respect to PHI, the Notice of Privacy Practices controls.

When you fill out a contact form, request an intro call, email us, or otherwise communicate through the Site, we may collect your name, email address, phone number, and any information you choose to include in your message. Please do not include detailed health information in website forms or email, as these channels are not intended for sensitive clinical communication.

Where a service provider handles protected health information on our behalf, we enter into a Business Associate Agreement as required by HIPAA.

Our Site is hosted on Squarespace and uses cookies and similar technologies to function properly and to analyze traffic. We use tools such as Google Tag Manager and associated analytics services, which may set cookies to collect usage information as described in Section 2.
